Good question. Everybody who wants to grow and weigh tomatoes officially should know how to access a form. Print a bunch of these out and do not wait for the last minute. Also, read and follow the GPC early tomato weigh off guidelines right above the early weigh off form link.

Go to the Giant Pumpkin Commonwealth (GPC) site: www.gpc1.org
Click on "Resources"
Click on "Early Submission Form for Tomatoes". Print them out and take one with you to where you weigh your tomato.
Also, click on "Early Tomato Guidelines" and familiarize yourself with it.
