When measureing for this how do you do it? do you pick up the pumpkin and put the tape underneith and then measure?

Josh. To measure the circumference of a Pumpkin/Squash you need to run the tape around the Fruit with the tape in a horizontal plane to the ground and around the widest sections of it. Run the tape around the widest point parallel to the ground. Larger fruit will require a helper or a peice of tape to hold the end of the measuring tape.

When measuring, what is the best method to use, the over the top method or the circumference method? Just wondering what the standard is.

Yup, the OTT, or 3 way as some folk call it, Measurement/est method is the best.

All 3 is the proper OTT method. Circumference + End to end + side to side. Bart Toftness wrote a good "how to" that is on this website.
